University of Virginia Medical Center Missing Data

A handheld device for on-call pharmacists used for University of Virginia Medical Center patients by Continuum Home Infusion went missing on October 5th.  The device may have contained patients’ names, SSNs, health ID#’s, addresses, dianoses, and prescriptions.  The hospital began notifying patients on Friday, November 30, 2012.  Those patients most likely affected received services August 2007 through September 2012. Under the terms of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), University of Virginia Medical Center is required to notify officals and patients.  ]]>
